John R.. Crouch Jr., M.D.

John R. Crouch Jr., M.D., is board certified by the American Board of Family Practice and has been practicing since 1967. Dr. Crouch attended Washington School of Medicine in St. Louis, Missouri, and completed his internship at St. Luke's Hospital of St. Louis. He served two years in the U.S. Army as a Flight Surgeon and one year as Combat Aviation Flight Surgeon in Vietnam, earning the Air Medal and Bronze Star. He completed his residency at San Bernardino, California's County Medical Center. He has held the positions of affiliate associate professor in Family Practice at UCLA and associate professor at Oral Roberts University's School of Medicine in Tulsa. Dr. Crouch is a member of the Christian Medical/Dental Society, Christian Medical Foundation, Tulsa Academy of Family Physicians, Oklahoma Academy of Family Physicians, American Academy of Family Physicians and the Society of Teachers of Family Practice.
